• Mail, email or fax completed forms back to Karen so they arrive at least 24 hours prior to your appointment.
• Payment is due at time of service. Payment may be in the form of cash, check, or credit card.
• For telephone consultations the new clients credit card information is collected at the time the appointment is made. Charge to the credit card is made only after the consultation has been completed.
• There is a 24-hour cancellation policy. If canceling within 24-hours of your scheduled appointment time, we reserve the right to charge you for this visit.
